Omni Community Health is looking for a trauma competent, quality-driven professional to join our amazing team as the Team Leader. As part of the Omni Family of Services, Omni Community Health is a behavioral health organization providing and coordinating care for children and adults. As the Team Leader, you would be responsible for supervising and directing the activities of the case management staff who are responsible for all aspects of service delivery for clients placed in their caseload. This position directs the work of others to ensure job performance follows Omni Community Health’s policies and procedures, OSHA, HIPPA, federal, local.

  • Ability to be a team member and thought leader.
  • Bachelor’s or master’s degree in social services, human services or related field required for case management oversight.
  • Minimum of three years’ experience working with developmentally, intellectually and/or physically disabled individuals or mentally ill individuals and one year of supervisory experience preferred.
  • Must be able to work independently and demonstrate good judgment in decision making; strong organizational and interpersonal skills required; or any equivalent combination of education and experience.
  • Valid Driver’s License.
  • Proof of Automobile Insurance and Reliable Transportation.
  • Verified state licenses, including credentialing and training, as required by state regulations.
